Can I be referred to Community Healthcare?
Yes, all you need to do is:
Obtain a referral from your GP
Your GP will refer you to CHL, you will then receive an E- referral letter with your procedure references.
Ring us to book your appointment
We'll take your details as well as your referral codes provided on letter to arrange a convenient appointment time and location.
Confirmation letter
You'll receive a confirmation letter with details of your surgical address, timings and any relevant instructions you may be required to follow.
